What a pinched nerve in the neck really is

Your cervical spine (the neck) has nerve roots that branch out between each vertebra and travel down into your shoulders, arms, and hands. A “pinched nerve” — clinically, cervical radiculopathy — happens when one of those nerve roots gets compressed or irritated. The culprit is usually a bulging or herniated disc, bone spurs from wear and tear, or a loss of the neck’s normal curve that narrows the space the nerve travels through.

Because that nerve serves a specific path down the arm, the symptoms show up along that path — which is why a problem in your neck can feel like pain in your elbow or numbness in your fingers.

The warning signs

A pinched nerve in the neck can feel like sharp or burning pain radiating from the neck into the shoulder or arm, pins-and-needles tingling, numbness in specific fingers, weakness in the arm or grip, or symptoms that get worse when you tilt or turn your head a certain way. Some people notice it most at night or first thing in the morning. If you have progressive weakness, or numbness in both arms, that warrants prompt evaluation.

Why posture is so often the hidden driver

Many pinched nerves in the neck trace back to alignment. When the head drifts forward and the natural neck curve flattens — the pattern we call forward head posture — the discs and joints get loaded unevenly and the openings where the nerves exit get narrower. Over time, that is what sets the stage for compression. It is the same mechanism behind a lot of chronic stiff necks and headaches.

This is why “just rest it” or a round of pain medication so often gives only temporary relief. If the alignment that is crowding the nerve does not change, the symptoms tend to come back.
